First, let me state there is no such thing as an industry standard. Your script only has to work for you and the creative team. That being said, I've found the following format to be very user friendly, to both write in and to read.

Page 1 (This is your page heading, and should be at the top of every page of comic story. When you get to Page 2, put in a page break. Don't hit the Enter key until you get to the next page, put in a page break. If you don't know how to do it, learn.)

Panel 1: (Your panel description goes here.)

Person A: (This character's dialogue goes here.)

Person B: (This character's dialogue goes here.)

SFX: (Your sound effect goes here.)

Then you rinse and repeat.

Another way to format is like this:

Panel 1:
(Panel Description goes here, underneath the label.)

Person A:
(This character's dialogue goes here.)

This is not the only type of format that can be used. There are others. It's just that this, or a derivative of it, is most often used.

This has helped me immensely. (Scroll down to Batman: The Hill and click on the link). There is also a visual aid from Kurt Busiek (floating around from years past) that helped out, too.
